The Osseo Secondary Transition Center is a program for students with disabilities (ages 18-20 ) who need assistance making the transition from school to life in the community.  OSTC guides young adults to be independent, contributing members of their community.

Phase is a ‘bridge’ between school and adult programs for young adults 18-21 years old with moderate to severe mental impairment and often one or more secondary disabilities. Phase emphasizes the transition areas of instruction vocational, home - living, community, leisure/recreation, and post-secondary; each student’s program is individualized based on his/her assessed needs.
